BSA Health Form
Per BSA policy, all Scouts MUST have this form filled out and submitted in order to camp with the pack. The form can be printed from your computer or emailed to Melinda upon completion. No information will be shared with any unnecessary parties. The information will only be used in emergencies or to keep your son safe (ie. avoiding food allergies, etc.). In addition, while forms are not required for siblings or parents, any food or insect bite allergies should be reported to Pack leadership in advance of trips.
